Hospital ward teams coordinate patient care through clinical tasks. Despite advances in electronic health records, task coordination remains largely broken — managed on paper lists and consumer messaging apps such as WhatsApp. The result is four significant and avoidable failure modes:
Lost Clinical Capacity
Junior doctors waste time rebuilding patient lists from data that already exists in the EMR. Constant interruptions for team alignment, and repeatedly switching systems to chase lab results and imaging, drain clinical capacity daily.
And when a clinician does have a spare moment, there's nowhere to look to find where help is needed. Tasks sit stuck while teammates are free. The team isn't slow — it has no shared picture of where the work actually is.

Clinical communication happens on WhatsApp and paper lists are lost in public spaces. Both are breaches under the Privacy Act 2020. This is not an edge case — it is the current standard of practice across NZ hospitals.
